You also shouldn't have a high bite on a brand new clutch. It should be the complete opposite, You should feel bite as close to the floor as possible, progressively getting higher as the clutch wears more. Usually means when the clutch was replaced, it wasn't installed properly for it to have a high bite, or your slave needs readjusting. The fact that your slave was replaced though still suggests the latter but I don't doubt it wasn't installed correctly either.
There's no doubt your clutch has failed.
On an aftermarket clutch you should have a lighter, more tactile throwout, with no noise increase during clutch movement. However during driving around at low rpm you might notice a bit of chatter, but you should have already disengaged the clutch completely before this even starts to happen.
If the endoscope *looks* fine, then chances are your throwout bearing has failed, which suggests a poor clutch install. Being an Exedy I think Knightrous is on the money though.
On my daily I've had the same clutch kit twice (Xtreme HD organic disc and plate on lightened flywheel). First install was great light pedal feel and noticeable noise under load in 2nd gear, but I didn't care since the clutch worked so well. Second time it was installed at the same dealership but a different person, unless I depress the pedal a cm or two, there's a slight squeal that sounds like belt slip. The throwout bearing is slowly failing as a result but I still have good clutch feel, and less noise under load this time around.
Luckily I still have the pressure plate and disc from my first install, when I know im up for a new clutch (or can afford it), I'll be sending it to NPC to recondition the pressure plate, and make me a better clutch disc using the original as a template, if they don't already have a replacement part number ;)
